Version 5 of the Core Assessment & Outcomes Package for Learning Disabilities was released in 2006 and introduced a fully integrated person-centred suite of documentation for learning disability services
This toolset integrates the processes of assessment and outcomes measurement and also includes care planning and review documentation. The package is now in wide use across the UK.
All elements of the package are both compliant with national guidance (e.g. Valuing People, CPA) and integrated with FACE toolsets for other care groups (e.g. Older people/SAP, Mental health/CPA), thus facilitating ease of use across health and social care agencies and full system integration when used electronically.
The Learning Disabilities Toolset can be combined with our Outcomes Software to provide a specialist Learning Disabilities client management solution.
