The tools are designed to be fully multi-functional, supporting assessment, resource allocation, and outcome measurement (both practitioner-based and PROMs). First and second line assessment tools, along with care and support planning tools, provide a framework within which the service user’s needs may be confidently met and recovery promoted.
The product of over 15 years’ development work involving hundreds of clinicians, version 6 of the Core Assessment & Outcomes Package for Mental Health Services, provides a robust system for the routine recording, reviewing and measuring of clinical data as well as facilitating service user engagement.
The toolset contains our nationally-recognised outcome measurement system and risk assessments and is designed to:
- Ensure accurate multi-professional assessment of needs
- Deliver person-centred, holistic assessment
- Promote recovery focused care delivery
- Support robust risk management
- Support crisis and contingency planning
- Enable accurate and efficient serious incident reporting
- Generate PbR Care Clusters
The toolset is fully compliant with the requirements of New CPA, provides integrated performance indicators, national indicators and MHMDS. HoNOS scores as well as Payment by Results care clusters can be generated quickly and easily from the assessments.
The toolset also integrates triggers for carer’s assessments, safeguarding, Caldicott and specialist assessments.
The toolset is applicable to all service users in secondary adult and older people’s mental health services. In addition to the general adult mental health population, the FACE tools provide evidence based assessments for:
- Young people and CAMHS services
- Mental Capacity and DoLS
- Substance misuse
- Perinatal psychiatry
- Chronic fatigue syndrome
- Forensic services
