The Core Assessment & Outcomes Package for Substance Misuse is a comprehensive documentation system for substance misuse services
It covers all stages of a person’s involvement with services, from referral to discharge. All elements of the package are both compliant with national guidance and integrated with FACE toolsets for other care groups (e.g. mental health), thus facilitating ease of use across health and social care agencies and full system integration when used electronically. The package embeds all NDTMS/NTA data requirements and also combines descriptive free text recording with the standardised measurement necessary to assess outcome and profile needs.
