The FACE Profiler is a client management system dedicated to the easy collection of person-centred information
Data collected using the Profiler can be used either to feed the FACE Outcomes Reporting suite or to meet documentation requirements through the use of print-outs and electronic templates. The Profiler is a functionally rich clinical workstation tracking all aspects of the patient journey including:
- Care planning – includes goal setting and care plans
- Incident recording – serious incidents, near misses etc
- Registration and referral
- Care pathways – recording and tracking of care pathways
- Diagnosis – comprehensive facilities for ICD-10
- Reviews
- Activity recording – recording of contacts, interventions etc
- Assessment – risk and CPA assessment using either local or standard tools
- Admission and discharge
The Profiler interfaces easily with existing administrative systems where this is desirable to avoid duplicate registration and data entry.
