Guaranteed to meet national standards
High quality assessment is the cornerstone of good practice. FACE assessment tools are nationally-accredited by the Department of Health and used throughout the UK & Ireland by NHS, social care and independent sector organisations.
The product of 20 years’ development work involving hundreds of practitioners, service users and carers, FACE Assessment toolsets are guaranteed to meet all clinical, management and national requirements. They are updated regularly with the involvement of our user network and we are constantly seeking to both improve existing tools and design new tools to meet emerging needs.
Each toolset provides a complete documentation system tailored to the needs of the relevant care group and includes both generic and specialist tools. These support:
- Holistic, person-centred assessment
- Multi-professional, multi-agency working
- Care and support planning
- Self-directed support and resource allocation
- Carer involvement
- Outcomes measurement
- Risk assessment and risk management
- Long-term conditions management
Paper and electronic use
FACE tools may be completed either on paper or electronically. Our clinical software enables easy collection, printing and analysis of all FACE tools. FACE tools are also available from most major health and social care software suppliers.
The FACE electronic library includes all FACE assessments as well as facilities for recording user-defined or standard assessment tools. The FACE library also includes other standard coding systems such as ICD-10 and DSM-IV. In total, the library currently contains over 50,000 data items covering all aspects of a person's health and social functioning, circumstances and care.
